Hello doctor,

I am 23 years old. I had a cyst removal in two sites on my lower lumbar area due to pain when bending. After the surgery, I was in more pain than before. I was treated with Kenalog, acupuncture, NSAIDs, and chiropractic care. It is five years of pain, and sometimes I feel numbness down my leg. I finally went for an MRI, and it shows disc desiccation and a herniated disc at L5. Please help.

I understand your concern and explain that your symptoms are related to the disc herniation and the sciatica nerve's irritation. Coming to this point, I would recommend consulting with a spinal surgeon and discussing surgery to help improve your situation.

Another possible treatment option would be steroid epidural injection to help improve the pain, but this is a definite solution to your problem, as the pain relief may be temporary.

I have seen the pictures (attachment removed to protect patient identity).

This seems to be a moderate severity as there is root nerve compression, but the spinal canal is not very narrow. I think that even epidural steroid injection in this region can help improve your situation, but it is not a definite solution to your problem. I think that another surgery may help reduce your situation.
