Hi doctor,

I am a 22-year-old and have not been getting periods for three months, but I had spotting this month with pelvic pain. Please help.

I thoroughly read your query and understand your concern.

First of all, do not worry about it if this has happened for the first time. I would like to know a few more details:

1. If you are sexually active, then the first thing you need to do is, rule out pregnancy.

2. If you are not sexually active or if the pregnancy test comes out to be negative, then I would suggest a few tests,

  • Complete blood count.
  • Thyroid profile.
  • Serum prolactin.
  • An ultrasound lower abdomen and pelvis.

3. There are many causes of delayed periods, and stress can be one of them. Stress can lead to changes in the hormonal milieu which can affect the menstrual cycle, so stop worrying about this.

4. If there is spotting, then maybe you will get your period in a day or two.

5. I suggest you have a course of Progesterone tablets to induce withdrawal bleeding after ruling out pregnancy, and if this recurs, then you will need further evaluation so as to make a proper diagnosis.

Thanks for the reply.

I am currently living abroad. I asked my doctor he recommended to consult a gynecologist. But I am not getting any appointment before July. Here I cannot even do any tests until my gynecologist gives me in prescription. I am sexually active, but I checked a couple of times, and I am not pregnant. Normally I get my periods after two months, but this time it is the third month, and I have had spotting for the last ten days. I am scared because of this spotting.

Do not worry about spotting if your pregnancy test is negative. Blood tests are just to rule out other causes of irregular periods. It is not an emergency. Did you take any emergency contraceptive pills in the past month? If yes, then do not worry about the spotting. It does not seem like an emergency to me, so even if you are not getting an appointment, you do not need to worry. Just wait for a week; you will get your period and if you do not, repeat a pregnancy test. Stop taking stress. If pain increases, then you can meet your doctor. Right now, it is not a gynecological emergency.

I suggest you do these things:

1. Stop getting stressed.

2. Start doing regular exercise.
