Hello doctor,

  • My father is 75 years old and he is overweight.
  • My father stays with my relatives in a remote place.
  • Last night when he was sleeping outside, the weather was cold due to rain and the fan was on.
  • He had felt cold at midnight and when he woke up, they found his right hand and leg were not working properly.
  • He can feel everything, he can also move both legs and hands, but feels lack of power in the right hand and leg.
  • He is not able to walk and not able to drink water.
  • They reside in a place where there is no neurologist.
  • They visited an ayurvedic doctor this morning and he has given some ayurvedic medicne and nurokind plus injection 2mL once daily. 

Please suggest the right treatment.

  • If your father had presented to me as an outpatient, I would have immediately taken him for a NCCT Head (CT scan of brain) as this is likely a case of stroke.
  • Ayurvedic medicines and nurokind will not help at this stage.
  • Immediately, get a CT scan of head done.
  • In the CT scan, if there is no hemorrhage (bleeding), that means if hemorrhage is ruled out, he should immediately be put on tablet Ecosprin 150mg per day and tablet Atorvastatin 20mg per day.
  • But, if hemorrhage is detected, these medicines must not be given as these can increase the size of hemorrhage.
  • So, hemorrhage should be ruled out first.
  • Physiotherapy should be started for the weak limbs.
  • It may take months to recover.

Best thing is to get a CT scan head done and share the reports as soon as possible. Delay should be avoided.

Also provide the below details:

1) Is he a smoker?

2) Does he have diabetes, high blood pressure or heart disease?

3) How many hours have passed since he developed this weakness?

Hello doctor,

  • We have done CT scan and attached the reports.
  • My father has BP problem, at the time of the incident the BP was 152.
  • He takes half a tablet for BP when required and when he has breathing problem.
  • He is not diabetic and does not have heart problem. 
  • About 30 hours have passed now, since the incident.

Hi,

Thanks for getting back with the CT scan head. (attachment removed to protect patient identity)

So, CT scan head has ruled out a hemorrhage. When it is normal, it is likely an infarct and the infarct takes time to show on a CT. I suggest you to get a repeat CT done after 48 hours and keep in touch.

However, repeat CT is just to confirm the diagnosis and it will not have any effect on the treatment. His treatment should be started as early as possible with the following medicines:

  • Tablet Ecosprin-AV 150/20 mg once a day (after food) for life long. In case of any surgery or internal bleeding in the body, this has to be stopped (just to mention, you should be aware of it that this medicine promotes bleeding).
  • Tablet Strocit-P (Citicoline-Piracetam combination) one tablet twice a day for 3 months.
  • Tablet Coversyl Plus one tablet twice a day.This is for high blood pressure. Meanwhile stop tablet Amlovas-AT (which he is currently taking) and start this medicine. His blood pressure should remain below 120/80mmHg in future, otherwise he will have more chances of strokes in future. If BP does not remain controlled on Coversyl plus, then tablet Amlovas plain (not amlovas AT) may be added. Please get back if BP is not controlled well in future.
  • Regular strict physiotherapy is important to bring back the function of affected limbs.
