Hello doctor,

My husband had retinal detachment surgery last year. He had a partial scleral buckle placed in his left eye. He has had a slow recovery but so far, no issues. Today, I hit him in the eye with my elbow. His eye is bloodshot and there is a faint color of purple. He says that he can feel a scratchy feeling but there is not any pain and his vision is normal. What should we do? What symptoms should we look out for if the buckle has been moved or is broken? He is now experiencing double vision. The double vision has subsided. Could you tell me what symptoms we should look out for if the buckle is moved or breaks?

Please help.

I went through your query and understood your concern.

You can see the buckle actually by asking him to look at all different peripheral gazes. The buckle will be seen as an elevated part in the otherwise normal conjunctiva. As for breaks you have to look for flashes of light if seen by him or any floaters. The blood you are seeing is subconjunctival hemorrhage. I suggest you use cold compression for that with vitamin C chewable tablets thrice daily.

His left eye is feeling better and the redness went down quite quickly. I just have one more question to ask. Does retinal detachment increase eye pressure and can increased eye pressure cause retinal detachment?

Retinal detachment causes low intraocular pressure (IOP) and low IOP can lead to choroidal detachment.

We visited the doctor, and everything was fine. I just have one final query. What are the chances of my husband's having retinal detachment again in the future. On his left eye, he had the partial sclera buckle and laser treatment to prevent future tears. He also had laser treatment on his right eye. What are the chances that he’ll experience retinal detachment again? Here is some pictures of his left and right eye. These images where taken yesterday.

As of now both his retinas are well lasered. There is no hole or tear. Just take precautions. No heavy weightlifting or strenuous work ever.

My husband recently got into a fight and was unsure whether his eye was hit. We went to the hospital, but the doctors there were quite rude and did not seem to care. I saw your scans (attachment removed to protect the patient’s identity). All is fine. It is a healed retinotomy there (red circle). The previous RD would have happened from that place and is well sealed now. retina is well lasered. Nothing to worry.
