Hello doctor,

Why is the foreskin of the penis inflamed? I have never had sex, but I have had balanitis once. I have taken FBC, PCR, and CRP tests, and all are normal. The only thing I use to wash my genital area is Demerol 500?

Please suggest.

If there is no history of any sexual intercourse, this appears to be a fungal issue. Avoid any steroid cream over the affected part. The diagnosis might change if there is a history of oral intercourse. I suggest taking the following medicines (consult with a specialist doctor, talk with him or her and take medicines with their consent),

1) Tablet Fluconazole 150 mg once daily for a week.

2) Clotrimazole cream to be applied twice locally for a week.

This should be the treatment plan.

The answer to that is no; I have never had sex. Could it be caused by Dermol 500 lotion? My doctor suggested tablet Isotretinoin, tablet Doxycycline 100 mg, and Fucidin H cream, Doxycycline cream. Could it be due to the washing powder or friction? Please suggest.

In that case, I am changing the diagnosis. This is most likely irritant contact dermatitis. This could be due to the ingredients of Dermol 500. This is not a serious condition. I will change my treatment plan too. I suggest applying mild steroid cream over this. Do not use any hard soaps to clean the part. Wash with water only. Which cream did your doctor advise?

I suggest (consult with a specialist doctor, talk with him or her and take medicines with their consent) anti-allergic tablet-like tablet Cetrizine (cetirizine hydrochloride) or tablet Levocetirizine for five days.

I do not know what is the cause? I use Dermol 500 lotion, and why does it flare up? I have been given Clotrimazole cream and Fucidin H. But to be honest, I have never really washed my foreskin when I was younger or older because I never knew it was required to. Saying that if I use soap, it also burns like crazy. The foreskin is also inflamed, so I have no clue what is going on. I am attaching a video; the foreskin is inflamed, and even the blood vessels are. Dermatologist gave me Dermol 500 lotion to wash with. I do not know if that is caused due to it or tablet Isotretinoin. My skin has been so sensitive lately after being on tablet Isotretinoin. What do you think is causing this? I have also got itchy, and the hair on the legs, the base is red I think it might be due to washing powder. Please suggest.

This appears to be irritant contact dermatitis. This could be any reason, like Dermol lotion, soap, or even clothes washed in hard detergents. To minimize exposure to an irritant, I have advised you to use 100% cotton underwear. This will reduce the symptoms. If you think this could be washing powder, then avoid using that powder for some days, and you should see the improvement.

So it will take four weeks to clear up? I can say that I have never really washed the foreskin as it was hard to pull back. Unfortunately, I washed it with soap, and I had burning sensation; even semen hurts. Does water irritate it? I have used Dermol 500 lotion and have noticed an improvement? Is this some sort of skin infection?

First of all, I think you should take the full course of Fucidin H cream and the anti-fungal cream. This should solve your issue. If the issue persists, I advise you to visit a dermatologist for an opinion and physical examination to diagnose. If the foreskin is not pulling back freely, that could be phimosis. This also should be confirmed by evaluation.
