Hello doctor,

I have had a tension headache for a full day. When I lay down I can hear my heartbeat in my ears. Almost like my hearing is pulsating. I do have pressure in my head. Is this normal? I feel pressure in my head when I stand and I am suffering from migraine. Kindly suggest.

1. Since when have you started to have headaches?

2. Do you feel that headache in which areas of your head?

3. Does this headache happen persistently or episodically? Was it episodically before and now turned into a constant headache?

4. If episodically, how long does one episode last? (from onset to complete relief without medication)

5. How frequent are these headache episodes? How many times in a week?

6. Do you feel any vomiting or feeling of vomiting or redness in the eye or watering from the eye or nasal congestion during a headache?

7. How is your day-to-day mood?

8. How is your sleep? How is your vision?

9. Have you noticed any trigger factors for his headache?

11. For what problem are you taking Metoprolol (Calcium Stearate and Cellulose) and Losartan (Titanium Dioxide and Hypromellose)? Is it for high BP (blood pressure)?

Please answer these for further suggestions. Thank you.

Investigations to be done

MRI (magnetic resonance imaging) brain, Ophthalmology examination to look for intraocular pressure and fundus, T3, T4, TSH (thyroid stimulating hormone).
