Hi doctor,
I have had testicle pain for the last three months since unprotected sex. My throat was red, which had now subsided. The intensity of neck pain has reduced but is still present. I have had constant testicle pain for the past three months. So, can you please interpret my ultrasound? I believe I got an STD three months ago but tested negative for chlamydia, gonorrhea, syphilis, and HIV. Please help.

From the ultrasound, you got right testicular inflammation, and on both sides, you have varicocele which is dilatation of your veins. Treating of inflammation can be done by taking antibiotics. I suggest you take- tablet Doxycycline 100 mg twice daily for a week, tablet Azithromycin 500 mg once daily for three days, and tablet Flagyl 400 mg (Metronidazole) thrice daily for seven days. For varicocele, treatment is by laparoscopic ligation, which can be done later. If you have any more queries, please let me know.

Do you know what kind of bacteria I may have? The medication you have prescribed should be taken simultaneously starting from today i.e.
Day1 - Two Doxycycline 100 mg, and one Azithromycin 500 mg, two Flagyl 400 mg.
Day2 - Two Doxycycline 100 mg, and one Azithromycin 500 mg, two Flagyl 400 mg.
Day3 - Two Doxycycline 100 mg, and one Azithromycin 500 mg, two Flagyl 400 mg.
Day4 - Two Doxycycline 100 mg, and two Flagyl 400 mg.
Day5 - Two Doxycycline 100 mg, and two Flagyl 400 mg.
Day6 - Two Doxycycline 100 mg, and two Flagyl 400 mg.
Day7 - Two Doxycycline 100 mg, and two Flagyl 400 mg.
Do you have any recommendations for my partner? Unfortunately, I am out of the station. Do you suggest I go home immediately to consult with a urologist regarding varicocele treatment?

The bacteria is likely to be Neisseria gonorrhea. You can go after your holidays regarding varicocele as this is not urgent. Please give the above treatment to your partner. And when you go back, I suggest you get a complete sexually transmitted infection screening test.

Hi doctor,
I completed day seven of your treatment plan yesterday. Unfortunately, I still have the same symptoms of a sore neck and pain in the testicle area. The pain in the testicles may be less than before, but the neck pain is still there. I want to reiterate that my urine tested negative for gonorrhea nucleic acid in a PCR test which should be highly sensitive. What do you suggest to me?

I am sure with antibiotics; you will be negative for infections. However, according to ultrasound, you got a bilateral varicocele, which can cause this residual pain, dragging sensation on walking, prolonged sitting, or exercising. Therefore, I suggest you consult a urologist near your area as the varicocele treatment is by ligation either laparoscopic or open microscopic. Further, for your neck pain, I suggest you see an ear, nose, and throat (ENT) specialist who can diagnose and treat it better. If you have any more questions, please let me know. Thank you.
