Hello doctor,

I am a 23-year-old male and in good health. However, I have a tight foreskin; when flaccid, I can pull it back fine, but when erect, I struggle, and it can sometimes make sex a little painful. I am against surgery and would instead treat it differently; please advise. The problem has been there throughout my life.

The condition you are referring to is called the tight foreskin. It is due to decreased elasticity of the foreskin in your case. The total relief is with surgical removal only. However, you can try the 1 % Hydrocortisone cream application on the edges and the undersurface of the foreskin twice a day for two weeks and get back to me. Consult with your specialist doctor, discuss with them, and take medicines with their consent. If you have the slightest improvement also, we can continue medical management. If there is no improvement, surgery is recommended. I hope you do not have diabetes. If you have diabetes, you need your blood sugar status before starting the treatment.
