Hello doctor,

I experienced difficulty retracting the foreskin and pulling it down over the glans penis (mild phimosis). I managed to do this with some difficulty for about ten minutes each night to clean the penis. I wish to avoid circumcision and understand that applying lignocaine gel may help. Should I apply it to the glans penis, the foreskin, or both?

Lignocaine is a local anesthetic. You should apply it to the area where you experience pain when retracting the foreskin, which is usually the skin itself. Avoid applying Lignocaine inside the foreskin due to your phimosis; external application is your only option. Given your age, it is important to rule out other causes of phimosis. Keep your blood sugar under control, take any prescribed antibiotics, and use a local moisturizing gel as well.
