Hi doctor,

I have had a non-healing ulcer for more than one year. I have taken many antibiotics. It healed to some extent, but not entirely. It is painless, and sometimes, few blood droplets come from the ulcer. What is this? Please help.

The concern you may be having may be due to multiple causes, including ulceration, which may be from post trauma or infection, which could be bacterial, due to delayed healing, or due to a condition like eczema, which can be genetic and may also be due to benign to malignant growth. For a confirmative diagnosis, I would highly recommend undergoing an in-person consultation for examination and testing purposes, which are done using the following:

1. Culture and sensitivity testing by taking a swab culture from the secretion from the ulceration.

2. By dermatoscopic examination, the doctor examines the skin closely to check for signs of any abnormal growth.

3. By biopsy, the doctor would take a part of the affected skin and test it under the microscope to see for abnormal cells.

These are essential for a confirmative diagnosis. For now, medications that will help symptomatically include:

1. Apply Mupirocin ointment thrice daily on the affected area.

2. Consume one capsule of Augmentin 625 mg after breakfast and one after dinner for five days to prevent infection.

3. Consume one tablet of Pantop (Pantoprazole) 30 mins before breakfast and one 30 mins before dinner for five days to prevent acidity from antibiotics and anti-inflammatory

4. Consume one tablet of Hifenac-P (Aceclofenac) after breakfast and one after dinner for three days. Make sure to eat well before consuming the tablet.

5. For excessive bleeding and delayed healing, dressing using paraffin dressing may be needed from time to time, which is performed by a doctor.

All in all, I recommend a direct consultation for close examination and testing to rule out other conditions and to start the appropriate treatment following a confirmatory diagnosis. Do not worry. With the correct diagnosis, they can be managed and treated. Do not delay.

Thanks for the reply.

I am worried and hope it is not some malignancy. One dermatologist said it is a Bowen disease, but she is yet to confirm. Tomorrow, I will be doing a biopsy.
