Hi doctor,

I am a 27-year-old female. My weight is 124 kilograms, and my height is 159 cm. I am trying to have a child. I have an absence of periods for the past three years. I get my periods only after taking a birth control pill (Yasmin). I have done my ultrasound scan. I have all the symptoms of PCOS. Please see my pelvic ultrasound images and say about the abnormalities seen. Please help me.

I have gone through the images (attachment removed to protect patient identity). Your ultrasound scan shows a bilateral polycystic ovarian morphology. However, the antral follicle count is not so good (less than four). Having a good antral follicle count of 11 to 30 in ovaries with less number of follicles allows you to try naturally. Try to have assisted techniques that support to have a natural conception. It results in a better outcome. Your uterus and endometrium look fine with no abnormalities. There are no signs of infection in the pelvis.

Does it seem like a hormonal problem? Do my fallopian tubes are blocked? What is the other way used to increase the antral follicle count? Does it indicate abnormal ovulation?

I have not seen your hormonal profile. But polycystic ovarian disease occurs due to a sedentary lifestyle which indirectly affects your hormones. So stay fit. Do regular exercise and eat the right food. Fallopian tubes are not dilated, and they do not appear blocked. The correct test used for checking the patency of tubes is hysterosalpingography. Antral follicle counts cannot be increased quickly. I recommend some assisted methods which can help you and plan the treatment. The scan shows that you have regular ovulation and your follicle count is low.
