Hi doctor,

I experience pain inside my vagina when I urinate. I also have stomach and lower back pain. Please help.

Thank you for your query.

It could be urine or pelvic or sexually transmitted infection. I suggest a urinalysis, urine culture, and a high vaginal swab. Serum beta-human chorionic gonadotropin can help confirm pregnancy if there is no menstrual history. A transvaginal scan to exclude pelvic pathology and a cervical smear will also be done.

  1. Strict hygiene.
  2. Use soft, comfortable undergarments.
  3. Use condoms to avoid sexually transmitted infections.
  4. Take a sitz bath for relief from symptoms.
  5. Drink lots of water and crane berry juice.
  6. If positive, have a transvaginal scan by expert hands and repeat beta human chorionic gonadotropin to check pregnancy progression. In case of pain and bleeding with a positive pregnancy test, report to tertiary care gynecology emergency.

    After collecting swabs, if a negative pregnancy test, I suggest you take a tablet of Doxycycline 100 mg twice daily and a tablet of Metronidazole 400 mg 8 hourly for 14 days. Then, consult a specialist doctor, discuss with them, and take the medication with their consent.
