Hello doctor,

I have had recurrent UTIs. The past month, I have had leakage. I leaked a little during bathing and tend to leak a little urine throughout the day. I do not have pain. I had just leakage. I did a urine analysis yesterday, and the result showed little bacteria and few WBC. Does this mean I have a UTI? I took a tablet Macrobid for this issue. Please help.

I understand your concern.

Bacteria in urine analysis means you likely have an infection as urine usually is sterile. Now I suggest you do a urine culture. I suggest you start tablet Augmentin 1 gm (Amoxycillin 500 mg and Clavulanic Acid 125 mg) twice daily, then follow up with culture results so we can assess further if you need to continue same or change to a sensitive antibiotic.

Hi doctor,

Thanks for your reply. They did a culture, and it came back as no growth. So I am confused about why they found bacteria and few white blood cells. The doctor put me on something for an overactive bladder, but I do not feel like I have that. I think that I may have an embedded UTI. Please help.

I understand your concern.

Does your leak increase if you cough or is there anything that increases your abdominal pressure? Do you have a leak associated with urgency, which means you will feel you need to go toilet immediately? Otherwise, will it leak? The first is stress incontinence, and the second is urge incontinence. These are two types and have entirely different treatments. Please let me know which type do you have. Another thing is if urine culture is negative, it means you do not have an infection. The bacteria might not be from urine but surrounding skin when you collect the sample, and that bacteria will not grow in the culture for urine. So there is no urinary tract infection.

Hi doctor,

Thanks for your reply. I will leak when I am taking bathing, like about 15 minutes after normal urinating. And sometimes, a little urine will come out about 30 mins after regular urinating. There is no pressure, though. Does it sound like I may have an embedded UTI?

No, you do not have UTI at all. Signs of UTI (urinary tract infection) will be urgency and painful urination. Leaking at occasions like this usually is a self-limiting condition and does not need treatment. If you want to know more about it, I suggest you do urodynamic testing to check bladder pressure, sphincter pressure, and leak point pressure. I think that test will make a clear picture and proper diagnosis. Then I can decide whether you need treatment.
