Hello doctor,

I am 26 years old and am experiencing some concerning symptoms. My last period was on the second week of last month. I have had period-like cramps extending to my upper back and abdomen for the past three to four days. The cramps are severe and very painful. Additionally, I am facing issues with constipation. I engaged in sexual activity ten days after my period onset date, and we are sure penis was withdrawn on time. My menstrual cycle is regular for about 32 to 33 days. I also noticed cloudy urine at times. I am diabetic, and I have some discomfort while urinating. Also, my urine has become more frequent for four to five days. I am currently taking Gluconorm SR 1000 three times a day. What are the possible causes and appropriate steps to take? Can you let me know if it is UTI?

Based on indications, there is more possibility of having a UTI (urinary tract infection) and an extremely small chance of having a ureteric stone. Pain radiating towards the back is a feature of ureteric or kidney stones. In a diabetic individual, the likelihood of infection is higher compared to a non-diabetic person. Based on the reports, I would suggest you do a urine routine microscopy test and start with a course of antibiotics. Have plenty of water and take the tablet Urispas (Flavoxate) twice daily for three days for pain. Consult your doctor, talk to them, and then take the medications as per their advice. If the pain is severe, you should do a USG abdomen (abdominal ultrasonography) and pelvis scan.
