Hi doctor,

I am a 32-year-old male. I had a scrotal USG for three months. It said that I have a varicocele of grade 1 with a pampiniform vein size of 2-3 mm. When I consulted a doctor, he prescribed some medicine, and I had it. But, the dragging pain persists. Yesterday, again I had a USG of the scrotum, and the report showed that I am normal with no varicocele. During the second time of USG, the radiologist insisted me to inhale stomach full several times and said that my pampiniform flex size is 15 and it can go up to 22. And, he stated that I was normal. During the first time of USG, the radiologist insisted on coughing by turning my head to the left side. My dragging pain is present after having a meal and prolonged sitting. I am attaching my ultrasound scan report. Please clarify the report.

From your reports (attachment removed to protect patient identity), the first radiologist found a mild form of varicocele and the second radiologist gave it as normal. If your pampiniform plexus measures 2-3 mm and with mild reflux, then it is borderline and the second report is normal. Please note that there is a possibility that you had some infection before two months due to which the veins might have appeared prominent, and there was a bilateral minimal hydrocele. When you had medicines, your infection got treated, and the veins were back to normal. In this case, there can be variation in findings of the ultrasound report. There is no medical treatment for varicocele. Therefore, if your recent report is normal, then the theory of having a mild infection holds good. I mean to say that probably you never had a varicocele and the finding was due to mild infection, and the veins were prominent. I suggest you get an ultrasound scan abdomen and pelvis with post-void urine done to rule out any other cause like kidney stones or urine infection. It can cause a persisting dragging sensation or probably a small hernia, which is not well visualized on the ultrasound scan. Please avoid wearing tight underwear clothes and do not do heavy physical activity for some days.

Your reports (attachment removed to protect the patient's identity) are fine in general. Your hemoglobin can go up if you eat nutritious food rich in iron and vitamins. It is important to know why you are taking Gabared Plus tablets and if you are having pain anywhere. Please eat a healthy 2200 kcal diet every day and drink two to three liters of water and fluids. Avoid anxiety because that can influence your health in a negative way. I hope your scrotum problems are under control.
