Hi doctor,

I have a swollen left testicle, and I am unable to achieve a full orgasm. I also experience a dull, aching pain in my left testicle and notice swollen veins on the left side of my scrotum.

A swollen testicle, dull pain, and dilated veins (which feel like worms below the skin) are signs of varicocele (dilated veins with a backflow of blood into the scrotum). This may require a Doppler sonography of the scrotum and, if confirmed, might necessitate a small surgery to tie the dilated refluxing veins. At times, prostate swelling may cause referred pain in the scrotum and issues with erection, orgasm, or ejaculation. This will require a rectal exam, basic tests, a sonography of the prostate, and, if necessary, a uroflowmetry test. You need to consult your family doctor first and then proceed with the workup and treatment. In the meantime, you can use scrotal support or tight underwear to support the scrotum. Try to avoid cycling or prolonged sitting if there is prostate swelling.
