Can black spots in the penis be some high risk infection?

Hello doctor,
I have noticed some black spots or black pimples for a while near my penis. I wonder what is that? I did my STD test, and it came out negative. The last time I had intercourse was three years ago. I did my test again after six months, and everything came out negative then, just for sure, I did it again after a year and it came out negative. I wonder what is that? It is not itchy, swollen, burn, or pain is present. I have attached the images.

Before concluding, I would like to ask a few questions, was any laboratory investigation done? Was Hepatitis B surface antigen (HBsAg) or Hepatitis C (HCV) tests done? Was any biopsy of the lesion done? The clinical picture attached (attachment removed to protect the patient's identity) appears to be sessile verrucae. It can occur because of old razors, unprotected sexual contact with a commercial sex worker, use of contaminated objects or lubes for shaving purposes. It can be treated by electrodesiccation with the carbon dioxide (CO2) laser technique. Multiple sessions are needed to clear the lesions. Maintain proper genital hygiene. Avoid sharing razors or contaminated clothes. Avoid sharing towels. Follow up for any doubts. Regards.

Thank you. Yes, I did HBsAg and HCV tests and they came out to be negative. This is not at high risk right? Is it curable? Will I ever get it again once removed?

No, it is not high risk. Instead, it is a hypertrophic scar (a scar that grows into nearby skin in the injury-prone area and never grows after one year). If it is not itching or painful, leave it alone. I would suggest an injection of Hydrocortisone acetate and 2 mL Xylocaine injection intralesionally into the entire scar tissue once a week for four weeks. Use a disposable five cubic centimeter syringe and 22 number needle. Use force to push the fluid into the scar tissue. Apply Heparinoid ointment or Povidone ointment in the scar tissue for two weeks to soften the hard scar. Consult your specialist doctor, discuss with them and take the medicines with their consent. If an incision is needed electrodesiccation with thermal cautery, refer local dermatologist. Post-operative care will also be needed to heal the lesion by anti-septic approaches. Follow up for further information and doubts. Regards.
