Hello doctor,

I have been suffering from an infection in my vulva for the last two months. I have severe itching, redness, and inflammation on the superficial skin, like urticaria or hives, but I am not sure what type of infection I have.

My doctor said that this was a fungal infection, and gave me a tablet of Fluconazole 150 mg, a capsule of Becelac PB for 15 days, and a tablet of Pan 40, but the symptoms had not subsided. Then, the doctor asked to repeat this medication for the next seven days, but there was no relief. So after that, I took treatment from alternative medicine for the last five days. The doctor said this was urticaria. However, I am not satisfied with any treatment. My blood sugar is 115 mg/dL. Please help me.

Your history is suggestive of a bacterial infection. It should also be ruled out. But till then, we can start prophylaxis therapy (preventive care). It is also to be evaluated for your sexual habits, and any recent or past infection of your male partner. If possible, follow up with a complete history. Till then, follow my suggestions:

1. Tablet Azithromycin (antibiotic) 500 mg once a day for three days on an empty stomach.

2. Tablet Ceftas (Cefixime) 200 mg twice daily for five days after the meal.

3. V-wash (intimate hygiene wash) solution for local application. Consult your doctor and take the medicines accordingly.

Yes, it is suggestive of bacterial infection in the vulva. I want to add another medicine to your treatment. Follow this:

1. Tablet Azithromycin 500 mg once a day for three days (To be taken on an empty stomach).

2. Tablet Ceftas 200 mg (Cefixime) twice daily for five days (after meals).

3. Tablet Atarax 25 mg (Hydroxyzine) once daily, continue for itching.

4. V-wash solution for local application. Consult your doctor and take the medicine accordingly. In addition, I recommend the following:

1. Maintain local part hygiene properly.

2. Avoid any sexual activity for at least one month.

3. Take regular medication and follow the suggested medicines. It will surely help you out.

You have got much improvement. But as the period comes, some physiological changes occur in the genital tract, and this has been causing the itching. So, continue using the medicines prescribed for another ten days. It will completely resolve your symptoms. Have you ever been sexually exposed?

1. Tablet Atarax (Hydroxyzine hydrochloride) 25 mg twice a day to be continued for itching.

I am sorry, but you need longer-duration antibiotics. Is it possible for you to take an injectable antibiotic for a single dose? Please follow the following medication:

  1. Tablet Metronidazole (antibiotic) 2 gm single dose for a day, for seven days.
  2. Tablet Secnidazole (Nitroimidazole anti-infective) 2 gm single dose for a day, for seven days.
  3. Tablet Atarax (Hydroxyzine) 25 mg once daily to be continued for itching.
  4. V-wash (intimate wash) solution for local application. Consult your doctor and take the medicine accordingly.

I will provide you with two alternatives:

1. Tablet Tinidazole (antibiotic) 2 gm.

2. Tablet Fluconazole (antifungal) 150mg. Consult your doctor and take the medicines accordingly.

Both are single doses. You have taken Fluconazole prior, but this time take it along with, Metronidazole or Tinidazole, whichever is available.

Yesterday I took all the single doses you prescribed (tablet Secnil forte 1 gm, tablet Metronidazole 400 mg, and tablet Atarax 25 mg) at night. Three tablets in the morning and two tablets in the evening. But swelling in the vulva is still present. Itching has reduced a little bit, but not completely. I am sending a picture with this. Please inspect the infected part, and guide me what needs to be done. I have taken all medicine continuously as you prescribed, but there is no relief. Please help.

Yesterday you took a single dose, so wait for two to three days. Drugs will do their work within a short period, you will find relief then. Till then, if you have itching., then only take the tablet Atarax. Otherwise, stop all medication for two to three days, and just apply V-wash lotion over there. How much percentage have you improved from symptoms? Is there any improvement?
