Can distal penile urethroplasty be done using EPA technique?

Hello doctor,

May I know if you have done urethroplasty in the distal penile urethra using the excision and primary anastomosis (EPA) technique? Are you personally familiar with any such case? If so, was the surgery successful? Did the surgery cause penile curvature? If so, how many degrees did it had? What was the stricture length? Thanks.

My phimotic ring is very tight. How can I make it less rigid and more elastic?

Hello doctor,

I am a 55-year-old male. Ever since I can remember I have been able to retract my foreskin fully, all the way to the base of my penis. I have always had a problem with the phimotic ring once it passes the head of my penis it is very, tight around the shaft of the penis which can be somewhat painful when the penis is fully erect. Is there any way to treat the phimotic ring to make it less rigid and more elastic? So, when it is around the girth of the shaft, it is not too tight.

I was advised to use Finasteride for four weeks before TURP. Is that safe?

Hello doctor,

I will be having prostate TURP surgery in five weeks. My Urologist has recommended daily Finasteride four weeks before to reduce bleeding during the surgery then stop. I am worried that Finasteride has been linked to prostate cancer. Is this true? Is there an alternative medication to reduce blood loss? I know the Finasteride is just for four weeks, but I am still concerned. Is there any research on short term use?

Is it mandatory to get prostate biopsy for PSA 5.7 with negative MRI?

Hello doctor,

I had a 4.4 PSA and had an MRI that was negative. Six months later my PSA is 5.7 and even though the MRI was negative it is suggested that I should have a prostate biopsy. I am a 73-year-old male. I am certainly not that excited to have a biopsy. Any thoughts?

I suffer from urine infection and the pain is reducing slowly. What could it be?

Hello doctor,

I am 35 years old. I have the symptoms of a urine infection. But five days after the beginning of the symptoms, I got a urine test, and no bacterial infection or STD were detected. I did an HIV test, which was also negative. The pain is reducing slowly, but I am still not back 100 %. I have no sex envy. What could this be? Could this be a chemical infection or allergy from using hydroalcoholic solution wipes or shampoo on the penis? Could this be a prostate issue?
