I have pain while passing stools and I have blood along with it. Sometimes, I have many blood spots on my stool. I consulted a doctor and did a physical examination. He told me that I have an anal fissure. He suggested me to take GTN 0.2% and tablet Brixen. If there is no relief in ten days, my doctor told me that I need surgery for it. Please suggest.

An anal fissure is a cut in the anal passage. It occurs due to the passage of hard stools. It presents with pain and bleeding in ninety-nine percent of cases. Surgery is not required in most cases. But proper medicines and prevention are necessary to be taken. I suggest you continue taking treatment as suggested by me. Have a follow-up after five days. If there is no relief, I will try to modify your treatment.
You can have a sitz bath with Betadine two times for 15 minutes. You can take syrup Duphalac (Lactulose) four tablet spoon daily. You can use ointment Diltigesic (Diltiazem) locally three times. Use ointment Smith (Lidocaine with Calcium dobesilate, Hydrocortisone, and Zinc) locally three times. Take tablet Enzoflam (Diclofenac with Paracetamol and Serratiopeptidase) two times daily and take tablet Pantop DSR (Pantoprazole) once a day.
Preventive measures:Drink plenty of water. Avoid taking spicy and fatty foods. Take a high-fiber diet.
Regarding follow up:Have a follow-up after five days.
