I have been suffering from back pain for the past 15 months. I am unable to stand due to pain. No medications helped me to reduce the pain. What could be the reason?

Thank you for your query.

I understand your concern. If you are not getting relief with medicines then you will require an MRI scan. Get an MRI scan done along with physiotherapy. I suggest you to follow a few instructions such as, avoid lifting heavy weights, avoid arching your back, instead bend your knees to pick up something from the floor, avoid sitting for long hours, avoid two-wheeler rides, improve your posture while sitting, try to get up from your chair every 20 minutes and take a quick walk around and then continue working. Always sit on a chair with back support, do not get up suddenly from a sleeping position. Instead, turn to the side and then get up, take bed rest if severe pain, do hot fomentations locally over the affected area – at least twice daily, take tablet Pregabalin 75 mg once at night for 10 days before food, take tablet Ibuprofen 400 mg twice daily for 10 days after food, take tablet Rabeprazole 20 mg 10 days before food, take a tablet of vitamin D (60k units), once daily for 10 days, then once a month for 11 months, take these tablets only if you are not allergic to them and do not take these tablets if you have bronchial asthma. Practice McKenzie's back extension exercises, back core strengthening exercises, glute strengthening exercises, and passive straight leg raising. These exercises can be done initially under a supervision of a physiotherapist. Start physiotherapy once the pain reduces. Only regular exercises can give results. After the pain reduces, the following activities are good for you such as, swimming, yoga, and walking. Jogging, running, and weight-lifting may aggravate back problems. Kindly consult a specialist, talk to them, and take medications with their consent.

Get an X-ray of the thoracic spine - AP and lateral views. Also, get an MRI of the thoracic spine with a screening of the whole spine done.
