Hello doctor,

I am 99 % sure it is balanitis, but to make sure, I really need your help. I have had the following complaints for the past month, white patches, dark reddish patches on the glans, a slightly foul smell, and itching on the foreskin sometimes. Kindly help me out with if it is balanitis. If yes, is it bacterial or fungal or any other? And also the best medication to get rid of it as soon as possible, as it is really bothering me both physically and mentally. Would highly appreciate your help, and thanks ever so much.

Yes, it is candidial balanoposthitis (balanitis). Inflammation or white patches with dryness is visible. The cause could be a yeast infection, improper genital hygiene, or not drying the area after cleaning. Use no soap cleanser or Episoft (Cetearyl and Cetostearyl alcohol) cleanser or Cetaphil (Hydrating Glycerine, Niacinamide, Panthenol) cleanser daily to wash genitals. Avoid scratching and rubbing of lesions.

I suggest you take the following measures and take the following medications after consulting with your physician:

1. Tablet Fluconazole 150 mg once a day after food for three days.

2. Tablet Levocetirizine 5 mg at night after food for seven days.

3. Apply Clotrimazole cream (Candid cream) at night for two weeks.

4. Maintain proper genital hygiene.

5. Wear loose, comfortable thin cotton inner wears.

6. Avoid occlusion, friction, and injury to the area.

7. Dry the area after cleaning.

8. Keep the area clean and dry.

9. Keep a check on sugar levels.

10. Avoid moisture, as moisture favors fungal growth.

Thanks for the reply.

I also have scabies, which I recently discovered. I got it from my workplace (healthcare sector). Will that make my balanitis worse?

I suggest you take the following medications after consulting your physician:

  • Apply five percent Permethrin cream at night on day one and day 7. Repeat another week.
  • Permite BB bar soap (Elestab) for the bath for one month.
  • Tablet Teczine-M (Levocetirizine and Montelukast) once at night after food for seven days.
  • Tablet Ivermectin 12 mg only once(repeat next week once).

Maintain proper genital hygiene. Balanitis can be associated with parasitic infestation too. Maintain proper hygiene. Wash clothes or inners in hot water and iron them. Avoid exposure to dust and pollutants. Avoid contact with pets or animal dander. I suggest you continue above mentioned treatment too for balanitis.
