Hello doctor,

I have small dark bumps around the crown of my penis. They have been present for years and cause no pain. I masturbate regularly, which seems to exacerbate them. It usually subsides over time without masturbation. One of the patches got bigger instantly after masturbation. Please help.

I have seen the image you shared (attachments removed to protect the patient's identity). It is balanitis with penile melanosis. The lesions seem suspicious. Biopsy of lesion needs to be done to rule out malignancy. Kindly follow these instructions till you get the test done. Avoid scratching and rubbing of lesions. Avoid rough handling. Wash the area daily with an Episoft cleanser. Apply Fucidin (Fusidic acid) cream on lesions twice daily for seven days. If there is itching, apply Candid (Clotrimazole) cream at night for three weeks. Maintain proper genital hygiene. Wear loose, comfortable thin cotton inners. Avoid occlusion or friction. I would also like to know if you have any family history of melanoma. Please follow up with the report.

I do not have any familial history of melanoma. I want to mention that the lesion had flared up during masturbation and then subsided over days. There is no itching, pain, or discharge associated with the lesion. However, it is hard to touch.

It would be best to get a skin biopsy to confirm where a sample tissue is removed from the site and sent away to a laboratory for examination under a microscope. I am suggesting that because the lesion seems suspicious on the penile skin. If malignancy is detected early, it can be treated with less scarring. Symptoms of malignancy are a non-healing sore that started as a patch or rash, a growth that bleeds easily and crusts over, change in shape or size of existing marks or discoloration, formation of ulcer or rough and scaly areas, and a raised area (elevated lesion). Risk factors include certain skin conditions, elderly population, dysplastic nevi, many moles on the body, unusual scars, weakened immune system, exposure to certain chemicals, and long-term sun exposure. You can do a self-examination and check for any changes. Also, it would be helpful to you if you followed these instructions. Avoid direct sunlight. Apply Photostable sunscreen daily at 9 AM to 12 PM to 3 PM and 30 minutes before outdoors. Use sunscreen lotion even on cloudy days. Wear protective clothing that covers your arms and legs. Avoid tanning booths. Avoid the mid-day sun. Take tablet Limcee (vitamin C) 500 mg once a day after breakfast for a month. Take Tablet Antoxid HC (antioxidants) once daily at night after dinner for a month. Take care.
