Hello doctor,

I am 28 years old and here are my blood test results:

HGB - 10.4 from 12.5 - 15.5; HTC 31.8 from 37-47; MCV 73.6 from 82-98; MCH 24.1 from >27; Iron Ferrozine FE-24.7 from 50-170; Ferritin- 3.29 from 5-1478; TSH 1.81 from 0.4-4; FT4 1.03 from 0.89-1.76; FT3 4.89 from 4-8.3; TGAb - 92 from 0-18; Anti-TPO 32.3 from 0-34; Calcitonin- 2.88 from 0-5.

I would be grateful if you could explain these results and help me to determine the diagnosis and suggest the treatment.

I can understand your concern.

You have not mentioned any complaint if present. In your report you can see low HB (hemoglobin), low MCV (mean corpuscular volume), low MCH (mean corpuscular hemoglobin), and low PCV (packed cell volume). This is suggestive of microcytic hypochromic anemia. It has four common causes:

1. Iron deficiency anemia.

2. Thalassemia minor.

3. Anemia of chronic disease.

4. Sideroblastic anemia.

As in your report, serum ferritin is low, which suggests iron deficiency anemia strongly. For still further confirmation do serum TIBC level which will be elevated and peripheral smear examination. You need to take Orofer XT- a Ferrous sulphate - containing iron tablet. for two months which will help replenish the blood. The cause should be searched by clinical examination like low nutritional intake, parasitic infection, vaginal bleeding, increased demand like in pregnancy, thyroid, or any other conditions. According to the cause, further specific treatment is given. Mentioning if you have any bleeding history, poor quality food history, abdominal pain, or stool occult blood history, if present can give better clarity.
