How to manage oral thrush and respiratory symptoms?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have a white fluid that goes up my throat and gives me breathing problems. It turns into some sticky glue and can dry up, looking like paper. I have bumps on my right hand that keep coming back and dry off after a few days. When it gets severe, it feels like needles in my hand.

Sometimes the left side of my abdomen hurts. Sometimes my testicles ache. I have had this problem for fifteen years. After kissing a dog at the age of sixteen, my mouth kept overflowing with white mucus that turned to paper when spit on the floor. Sometimes during the night, strings of mucus would move in my mouth.

One night, I managed to pull a string out with my right hand. I saw on the string two small, two-millimeter-long worms moving on my right finger. Then, in the following days, I developed a giant rash of bumps on my right hand. More symptoms followed, such as abdominal pains coming from the right side and testicle pains in my private area.

Please help.

Hello,

Welcome to icliniq.com.

Ok, you can purchase that Albendazole kit, but you do not know why? Get your stool examination done for ova or parasites to see if you have intestinal parasites. Sometimes the string you pulled from the skin might be a fiber, or due to damaged hair or skin. You can get a skin scraping examination done to rule it out.

With regards to oral fluid, it might be a case of possible oral thrush. Get an oral swab culture done to rule out yeast infection and treat it adequately. I can only suggest taking,

1. Probiotics early in the morning for at least ten days.

2. Maintain good oral hygiene.

3. Get checked by the dentist for a possible bad tooth.

4. Take antacids to stabilize stomach pH. Get an upper endoscopy done to rule out stomach issues.

This is the only way you can find what you have. Discuss this further with your caring doctor.

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.

Patient's Query

Hello doctor,

Thank you so much for the reply.

I will give you the raw details as I am desperate. When I was 16 years old, my friend's dogs had hookworms and I caught them. The dogs kissed me a lot on the mouth, and one day, my spit became very thick and started to choke me. I had a lot of mucus reflux.

Most nights, a strange string of saliva would go down my throat. One night, I finally managed to pull it out. I then saw two small hookworm larvae on my hand (white worms, about two to three millimeters long, burrowing aggressively on my right hand index finger).

The following days, they had burrowed into my skin, causing massive rashes on my right hand. Unfortunately, I had bad parents, and despite showing them the rash, they refused to help me and got mad at me for spitting out a lot of mucus everywhere in the house, which I was forced to do, or I would choke.

Today, I am 31 years old. The symptoms have diminished. The rash on my right hand sometimes comes back, and the mucus sometimes chokes me while I am sleeping or jogging outside. Other symptoms exist, such as strong abdominal pains or testicular pains, which also started a bit after the infection.

When I ask for tests, I usually get blood or urine tests. Unfortunately, they show nothing, and I stay stuck with this problem. I refuse to engage in a relationship while displaying this infection and symptoms, and spend my days looking for help to get rid of it.

Please help. Thank you.

Hello,

Welcome back to icliniq.com.

I can understand your concerns.

First, I do not see why you should restrict yourself from being engaged in a relationship. Although you might have parasites, that is not your fault and not a valid reason for staying alone.

I suggest you do a stool examination to confirm parasites, especially ova parasites.

The testicular pain could be due to:

  1. Masturbation especially if you involve hard objects to masturbate.
  2. Any local trauma especially when being young.

With regards to sputum problems, it is common for people to get sick with viral infection (common cold, flu) or with a bacterial infection like strep throat. A simple strep test can determine the right diagnosis. If nothing, then acid reflux to ruled out through upper endoscopy.

Again there is no reason not to get engaged although having such problems. Please get back with your laboratory reports.

Thank you.

Patient's Query

Hello doctor, thank you for the reply.

There are no hard objects or any suspicious activities of any sort. I just live alone and try to take care of my apartment. I am still experiencing symptoms of parasites such as rashes, abdominal pains, and mucus reflux. Could you please tell me if I can get that Albendazole kit? I read albendazole could get rid of them. In the meantime, I will try to go get those tests you have mentioned.

Thank you.

Hello,

Welcome back to icliniq.com.

I still think you need to run the stool examination first to understand if you have intestinal parasites or just skin manifestations of eczema. If parasites are present, you can get that Albendazole kit. Usually, in such difficult cases, we advise taking fecal samples for three consecutive days and sending them to the laboratory for examination. Even if only one of the samples is positive, definitely you have parasites, and get an Albendazole kit.

Thank you.

Patient's Query

Hello doctor, thank you for the reply.

I was able to take pictures of what seemed like hookworms under my microscope. I also caught a new disease called paronychia. While working at my old job as a packer for a supermarket, I started having wounds on my hands that would not heal (around the fingernails). Please suggest.

Thank you.

Hello,

Welcome back to icliniq.com.

Was the sample from stool?

Do you have pictures of other microscope views?

Have you been self-examined or sent to a local laboratory?

The one you sent in was similar to hookworm but sometimes artifacts can mimic the parasite. That is why I need the information as above.

Paronychia is biting of nails and this is mainly due to local trauma from biting and is more psychological. These continuous traumas have damaged the structure of nail beds. This makes it easier for bacteria or fungi to invade the nail bed leading to continuous infection and not healing properly. So you should stop the biting habit. And start local therapy with antibiotic ointment and antifungal nail lacquer.

Thank you.

Patient's Query

Hello doctor, thank you for your response.

The hookworm pictures came from my spit. When I was younger, I had terrible build-ups in my mouth that would nearly choke me with a massive rash on my right hand. Due to bad parenting, I was not given the required treatment for it. I was meant to give this microscope slide to a clinic doctor who had requested me to show him visible hookworms. Unfortunately, once I was ready to show him, he refused to look at it claiming he did not want to be proven wrong and that the disease is too rare in my country.

It displays four visible red hookworms on it with a fifth one that has a claw-shaped mouth.

As for the paronychia, it happens on its own. I do not bite my fingernails. After working as a packer, I started to have wounds on my hands that would not heal. The skin over my fingernails raises and the skin just peels off causing wounds that bleed and leak for days.

As for the stool tests at the hospital, due to complications, I was requested to redo the test three times and eventually got busy and had to put it to rest.

Hello,

Welcome back to icliniq.com.

So far, this is what we can do. Although you might be right, I still suggest you redo the testing for three consecutive days. You can send it to any local laboratory not only a hospital. We do encourage you to be healthy, and keep appropriate personal hygiene. You can use natural remedies like garlic (mix one chopped garlic clove put in a small cup of milk, and drink it early in the morning). Problems with nails should be resolved with antibiotic ointment and antifungal nail lacquer.

Thank you.
