Hello doctor,

I am a 33-year-old male. Two weeks ago, I engaged in unprotected intercourse with my girlfriend for the first time, and now I am experiencing itching and a painful, soft, red bump on the head of my penis. I am worried this might be a sign of a sexually transmitted infection, including HIV. What steps should I take to get tested for HIV, and what are the chances of contracting such diseases? My girlfriend appears to be in good health with no issues.

Please help.

Kindly share a clear image of the affected area. If unprotected sex has occurred, and you suspect the possibility of an infection from your partner, I would suggest you take the following tests:

  1. TPHA (Treponema Pallidum Hemagglutination).
  2. HPV DNA PCR test (human papilloma virus).
  3. Herpes Simplex Virus (HSV) IgG for both type 1 and 2.
  4. Herpes Simplex Virus (HSV) IgM, serum.
  5. Chlamydia Trachomatis IgG.
  6. HIV-1 and HIV-2 antibodies, screening test.
  7. HbsAg (Hepatitis B surface antigen).
  8. A urine complete test.
  9. If an infection is not confirmed and your symptoms may be due to a simple bacterial infection, I would suggest you do a local application of T Bact (Mupirocin) ointment and a course of Doxycycline (100mg) twice a day for five days.

    If itching is a concern, consider the possibility of a fungal infection, and I suggest you use Candid B (Clotrimazole and Bexclometasone) ointment for local application and Cetirizine (Cetzin) tablet once a night to alleviate itching.

    I would also suggest you maintain good hygiene by washing your genital area twice a day with normal water at a moderate temperature (neither too hot nor too cold). Avoid using strongly pH-altering soaps.
