Hello doctor,

I have noticed the presence of some bumps on the head of my penis. These bumps have been there for several months, possibly even years, and I have recently observed that there may be a slight increase in their number. Importantly, I do not experience any pain, itching, or discomfort. The only concern is their visual presence. While I have a history of herpes, these bumps do not appear to be typical of a herpes outbreak. If they are not indicative of a serious issue requiring treatment, I would appreciate guidance on how to address or remove them.

Based on the images you have provided (attachments removed to protect the patient's identity), the bumps appear to be consistent with Fordyce spots. These spots are sebaceous or oily glands, and they are entirely harmless and not sexually transmitted. They can manifest in various areas, such as the inside of the cheeks, the vermilion border of the lips, the glans or shaft of the penis, or the vulva in females. The size and number of Fordyce spots may or may not increase over time. The only available treatment, if desired for cosmetic reasons, is to have them treated with a laser. Dermatologists or urologists who offer laser treatments in their practice can perform this procedure. Fordyce spots are harmless, and the decision to treat them is based on cosmetic concerns.
