Hello doctor,

I have been experiencing persistent urinary issues for the past year and a half, but I have not found relief or gained any insight into the problem. My main symptom is a burning sensation while urinating, accompanied by increased urgency and poor flow shut-off when it worsens. I initially saw a doctor who thought it was a urinary tract infection (UTI) and prescribed seven days of Cipro, which was ineffective. He then suspected prostatitis and put me on a 21-day course of Cipro, but that also did not help either. I sought the expertise of a urologist who prescribed Ceftin for 30 days to test a different antibiotic, but it did not improve my symptoms. The only thing that seems to keep my symptoms under control is taking several drops of concentrated Potassium Iodide daily. If I stop taking it, the symptoms quickly return. The urologist performed a CT scan, which showed mild enlargement (10 to 15 percent) of my prostate. A cystoscopy and injection of an IC cocktail into my bladder revealed nothing unusual. Frustratingly, the doctor admitted not knowing what was causing my symptoms and suggested a TURP surgery as a last resort. However, I am not satisfied with the quality of care I receive from this doctor, and I believe there might be other options to explore.

I recently had a blood test that showed low testosterone levels (around 267), and the doctor's only recommendation was to take Clomid to try and boost the levels. However, he did not express concern about the cause of the low testosterone or if it could be connected to my existing urinary issues. I feel the need to understand why this is happening. Are there any possible reasons a healthy 38-year-old male would have low testosterone? Could it be related to prostate issues? Are there any additional diagnostic tests I should consider to clarify my condition? I will attach my medical records from this physician for your review. In your case, undergoing a TURP (transurethral resection of bladder tumor) procedure may not provide benefits and could lead to permanent retrograde ejaculation. Therefore, I do not recommend prostatic surgery to solve your condition. The absence of structural problems is reassuring, indicating no serious cause for concern. Regarding your symptoms, antibiotics will likely provide any relief or improvement.

Treatment plan

For treatment I suggest you take: 1. Tablet Tolterodine (tolterodine tartrate) 2mg at bed time. 2. Tablet Tamsulosin (tamsulosin hydrochloride) 0.2mg at bedtime. 3. Tablet Pregabalin (gamma-aminobutyric acid) 75mg at bedtime. 4. Psyllium husk (plain) three tsp at bedtime with two glasses of water over it.

Preventive measures

Preventive measures may include doing pranayam. This will help you isolate and control your perineum.
