Hi doctor,
I am a 30-year-old male who has been experiencing inconsistent urine for five years. I always have the urge to urinate, and there is a burning sensation. I feel there is a weak urine stream and difficulty with the flow. I could not sleep for seven hours and had to wake up two times. I did a lot of masturbation for six years. I have a problem of early discharge. I think the infection is spreading to the body, and there are lumps on the hands and thighs. I got tests done, such as a complete urine examination, and traces of glucose were found. There was a weak urine flow during uroflowmetry, and no other abnormalities were found. Please advise. Is this prostatitis?

Have you ever had tuberculosis or been in contact with someone who had active tuberculosis at that moment of time? Have you ever had blood in your urine? How much urine do you pass during the day? Do you have to strain to pee? Do you have any weight loss or pain in your sides? Have you had any spinal injuries? Do you suffer from constipation? When you took Mirago, did it help? Who prescribed it to you?

There was no sign of blood in urine till date. I can hold urine. There is no issue of an overactive bladder. Sometimes I have to strain to pee but not always. Urine flow is not constant; it stops and starts, and there is no spinal injury. Yes, constipation is a problem, but it comes and goes. A couple of months back, there was blood in my feces. I do not leak urine. One of the urologists advised Mirago and Levofloxacin. Another urologist said it was chronic prostatitis and advised me to take medicine for one month, but I did not take it for one month. At last, cystoscopy is advised, but I do not want to insert anything in the bladder at this age. My sugar is normal. My problem is I feel an urge to urinate every one to two hours and a slight burning sensation all the time. I also have heaviness in my chest.

I understand your concern. It appears that you have chronic prostatitis (inflammation of the prostate), and it is different from benign prostate hypertrophy, which happens in men older than 50 years. If you follow the advice I mentioned below, there is an 80 percent chance that the problem will disappear. However, there can be remissions in some patients. I suggest you follow the general advice: Have 0.79 gallons of fluids per day. Avoid too much tea or coffee. Do not smoke if you do. Masturbate or have sex at least two to three times per week. I also advise you to take the medications for three months and also consult a specialist and take consent for the medications. Take capsule Urimax (Dutasteride) 0.4 mg, after taking the first dose, you may feel lightheaded or dizzy, it will stop after one day, tablet Cranpac (cranberry extract) once a day, tablet Econorm (contains saccharomyces boulardii) 250 mg twice a day, tablet Chymoral forte (Trypsin-chymotrypsin) twice a day, capsule Doxy (Doxycycline) 100 mg once a day for one month, and tablet Ciplox (Ciprofloxacin) 500 mg twice a day for 14 days.
