Hello doctor,

I am a 54-year-old female with a history of smoking and alcohol consumption daily for 20 years. I have left smoking for the last seven years and alcohol intake for the past two years. I have been scheduled for a cardiac MRI stress test in the coming day, and I have heard that Adenosine injection can cause heart failure in every one in 1000 patients. What if I have an underlying cardiac disease, and the injection might cause a problem? I have undergone four CT scans due to COVID-19, and more radiation from scintigraphy might not be appropriate. Is there any alternative test for this? Is Adenosine dangerous for me? Kindly help.

I understand your concern. Why is a stress CMR (cardiovascular magnetic resonance) test advised? Did you have chest pain? Stress CMR is done to assess cardiac muscle viability and function and to see previous heart attacks. Adenosine's side effect is that it can cause shortness of breath in those with asthma. Another side effect is that it slows the heart rate. But Adenosine is infused in a controlled setting, and after stopping the infusion, the heart rate returns to normal within a minute or two. So there is very minimal risk associated. Kindly revert with reports. Thank you.

I understand your concern. T wave (attachment removed to protect the patient’s identity) is the most non-specific wave for ischemia assessment. T waves can be flattened in low potassium levels in the blood. It may also be a normal variant. ECG (electrocardiogram) is always interpreted in the context of clinical history. Regarding your query about Adenosine, it can precipitate asthma in asthmatics. It can slow down the heart rate for a short time. In the case of ischemic heart disease, it can cause chest pain. The effect can be reversed with Aminophylline injection, which is available with Adenosine always. So there is nothing to worry about. Thank you.
