Hello doctor,

My mother started experiencing severe eye pain earlier today. She had cataract surgery in that eye about three months ago. It gets worse when exposed to light (but still hurts constantly with the eye closed). She also reports an increase in floaters that started a few days ago, described as many small "squiggly" lines that appear when she opens the eye (but fade away after a few seconds). No sudden flashes of light, shadow "curtain" over the field of vision, increased blurriness, or other visual changes- the pain is the main thing (other than the floaters). No nausea or headache. What could be the cause of this? What should she do for pain relief/any other treatment? Is it possible this is an emergency, or does that not sound likely? It has also been unusually bloodshot for a few weeks, and intermittently painful (severe pain only started today).

I read your query and understand your concern. Can you please upload images of her eyes? Is there any fogging or blurring of vision and is it a throbbing pain or a dull aching pain? I need images mainly with close-ups and a flashlight. So I can guide you better.

Hello doctor,

She describes it as a "sharp overall ache" that gets worse when exposed to light. This does not sound like there is new fogging or blurring; she mentioned close vision has not been very good in that eye for a while but no major new change today. Sounds like the pain has improved somewhat after taking Tylenol a few hours ago.

I read your query and understand your concern. As I can see she has circumcorneal congestion. It can be the following things, either a corneal abrasion most likely as there is photophobia, or an acute uveitis attack (less likely as no blurring of vision). I am sending a list of medicines if you can get them from the pharmacy.

1. Eye drop Moxifloxacin 0.5 % four times a day.

2. Eye drop Sodium Hyaluronate 0.18 % four times a day.

3. Eye drop Cyclopentolate 2 % at bedtime.

4. Eye ointment Chloramphenicol and Polymyxin B at bedtime.

Start these and she will be fine, a pain killer you can take. I hope this helps. Feel free to get back for more queries.
