Hello doctor,

I am waiting on an initial appointment with my urologist but wanted to get some more insight into conditions I may have if possible. For the past 8 months I have experienced chronic, dull and sometimes mildly sharp pain in the testicles that seem to be the result of a chlaymidia infection I left untreated for a year before this. When I first contracted the STD, I experienced penile discharge and painful urination until those symptoms went away on their own. After that I experienced an itchy feeling in the scrotum for a few months until it became an achy tender feeling in the right testicle and shortly after in the left. Late last year, I finally went to seek treatment and received a round of azithromycin to treat the infection but it did not do much to relieve the pain. After this, I began a 10-day treatment of Doxycycline and while it did relieve the pain and tenderness, shortly after I finished off that round of antibiotics the pain and tenderness came back. After this I tried a 20 day treatment of Doxycycline to about the same effect.

I can feel the epididymis gets swollen at random times especially after ejaculation. Ejaculation also sometimes results in extra pain in the testicles or epididymis. I have found pain relief in wearing supportive underwear and taking Ibuprofen. I have also noticed it is more difficult to get and maintain an erection. Also, while I do not notice pain in the prostate, there seems to be less force behind my urination and sometimes I have a weak stream. While I believe this started with an initial chlaymidia infection, I have recently tested negative on a urinalysis for chlaymidia, along with gonnorhea and trichomoniasis and I have not been sexually active since that test. Based on the information provided, does it seem to indicate chronic epididymitis? Is it possible a bacterial infection remains in my epididymitis or testicles even with a negative uranalysis for these STDs? Could there be other complications also contributing to these symptoms? Also, is it possible that this infection has already caused permanent, irreversible damage to the epididymis, testicles or other systems related to it due to the length of the infection or otherwise? Should I be concerned with fertility issues? Also, how likely is there to be permanent solution to these issues I have stated?

Thanks for your detailed description and test report provided (attachment removed to protect patient identity). After reviewing all actually STD might be recurrent and chronic as well. In male prostate gland can get infected and will need weeks of treatment as prostate have a capsule which is difficult for the antibiotic to penetrate and cure. The negative results of swab are not enough as your urethra will have no more organism but the infection can be in prostate and cause long term issue. Long term treatment with Ciprofloxacin 500 mg for 6 weeks after getting semen for culture can be effective in addition to alpha blocker like Tamsulosine 0.4 mg once at night might be required to relive symptoms as it acts as a smooth muscle relaxant. Repeat of semen culture is required after the Cipro treatment of 6 weeks. The problem might be reoccurring again as chronic prostatitis is difficult to treat and might be converted to abacterial chronic problem (means without infection) different modalities of treatment then must be given with varying response. So as for now you will need semen for culture and start Ciprofloxacin 500 mg twice daily and Tamsulosine 0.4 mg once before bed for 6 weeks then repeat semen culture if the initial result was positive. This issue does not really affect fertility unless there is another issue of primary infertility or varicocele.

Yes, treatment covers testis, prostate, and genitalia and recommended for STD as well. For the pain you can use Ibuprofen 400 mg twice daily or Naproxane Sodium slow release 500 mg twice daily with coverage of tablet Omeprazole 20 mg before bed to prevent you from gastritis in this period. But before starting antibiotics, please do culture test.
