Hello doctor,

I had protected sex with a masseuse in a spa. I was using protection all the time and I am 99 % sure it did not break because when I pulled out myself my semen was still inside the condom. Now in the third week, I am experiencing a sore throat and little swollen lymph nodes. I am very worried about these early symptoms of HIV.

I got tested with CMIA TEST ( HIV 1/2 + P24 ANTIGEN TEST) 18th day and 24th day and both came out nonreactive. Does this test detect p24 third antigen in a week? I am really worried and need your advice.

I can understand your concern

I have seen your reports (attachments have been removed to protect patient's privacy) which are nonreactive for HIV (human immunodeficiency virus).

The CMIA (Chemiluminescent Microparticle Immuno Assay) like a combo HIV test detects both antigens and antibodies for HIV and is hence considered a good diagnostic test.

It conclusively rules out HIV after the third week. So better to test with CMIA after four weeks of exposure in my opinion.

If this test is negative after four weeks of exposure, then again testing is done with CMIA after three months of exposure to rule out HIV conclusively can be done.

HIV viral load can be detected by RT PCR (Real-time reverse transcriptase-polymerase chain reaction) test after two weeks of exposure. To rule out HIV just now at around three weeks then get tested with RT PCR or western blot test (but this test is costly).

If you can't afford such a costly molecular test then repeat CMIA or generation ELISA (enzyme-linked immunosorbent assay) after the fourth week and again after three months.
