Hello doctor,

I am having a mild headache, weakness, dizziness, and L4-L5 disc bulge, and my CPK value is currently high. I used to go to the gym before. My CPK values were 650 this month and 630 last month. I resumed walking. Last Monday, I did a 10 km walk, and I was exhausted and drowsy for three days since then. I stopped my exercise and got my CPK test done two days ago, and it is 294. I am unable to sit and work without pain. I feel head pain even when lying down.

CPK (creatinine phosphokinase) is a muscle enzyme. It can get increased in many conditions. The normal physiological conditions where CPK has been found to be raised are heavy exertions, sports activity, intramuscular injections, etc. The common diseases associated with high CPK are cardiac and renal, autoimmune diseases, endocrine diseases, vitamin D deficiency-associated myopathy or muscle disorders, or drug-induced myositis.

Presently you need evaluation for your fatigue. If you have features of muscle weakness then consult urgently The investigations advised are as follows-

1. CBC (complete blood count).

2. LFT (liver function test).

3. KFT (kidney function test).

4. CRP (C-reactive protein).

5. Thyroid profile.

6. Sugar (fasting).

7. CPK and LDH (without heavy exertion for a week).

8. Urine for RE.

9. ANA (by IFA method).

10. 25 (OH) vitamin D3.

Many of the causes of this fatigue, exertion, and muscle weakness are covered under these tests. Please review after test results.
