Patient's Query
Hello doctor,
I had noticed red discolored patches on the glans penis three months before, without any itching or fluid discharge. I visited a general physician. He prescribed me to take Itraconazole capsules and Ketoconazole cream for seven days, and I did not get any benefit from it. I again repeated the capsule regimen for 20 days, and I have been applying the cream twice a day for two months. I have only 10 percent benefit till now. Can you please suggest to me what to do now?
Kindly help.
Hello,
Welcome to icliniq.com.
I understand your concern.
Red discoloration on the glans penis that has been present for three months, without itching, burning, discharge, pain, or scaling, and that has not responded to prolonged antifungal treatment, is very unlikely to be a fungal infection.
True candidal balanitis usually causes redness with itching, irritation, burning, or whitish discharge, and it typically improves within one to two weeks of proper antifungal therapy.
Since you have taken oral Itraconazole twice and have been applying Ketoconazole cream for two months with minimal improvement, continuing antifungals is unlikely to help and may even irritate the sensitive skin of the glans.
Common non-fungal causes of persistent red patches on the glans include:
Irritant balanitis (from soaps, friction, excessive washing, or antiseptics).
Zoon’s balanitis (plasma cell balanitis).
Early lichen planus.
Fixed drug eruption.
These conditions do not respond to antifungals because they are inflammatory rather than infectious. Overuse of topical creams, especially in the genital area, can sometimes worsen redness due to irritation.
At this point, the appropriate next step I would suggest is to stop antifungal medication and shift focus toward gentle care. Use only lukewarm water for washing (avoid soaps, body wash, antiseptics, or scrubbing). Keep the area dry but avoid excessive friction. Apply a bland, fragrance-free moisturizer or plain petroleum jelly in a thin layer to protect the skin barrier.
If the patch is sharply defined, shiny, persistent, or not improving after appropriate anti-inflammatory treatment, you should consult a dermatologist rather than a general physician.
A proper examination is needed to rule out chronic inflammatory conditions. If you are diabetic or have not checked your blood sugar recently, it is also reasonable to screen for diabetes, as recurrent genital redness can sometimes be associated.
Most importantly, since there is no pain, ulceration, discharge, bleeding, or progressive worsening, this does not immediately suggest anything dangerous. However, because it has persisted for three months, a dermatology evaluation is appropriate rather than continuing antifungal therapy indefinitely.
I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.
Thank you.
