Hello doctor,

I am using Hydroquinone, Mometasone and Tretinoin cream for five years now, after applying this cream my skin tone became fairer. But when I did not use it for two days my face started looking darker. I also have pimples and dark spots. Kindly suggest which cream to use.

I have read your query and understand your concern. Pimples are commonly caused due to oily skin and sometimes hormonal changes. I suggest you follow the below instructions:

1. Do not use hair oil and sticky cream on the face.

2. Avoid oily food, chocolates, and milk products.

3. Avoid dust, and use sunscreen with SPF (sun protection factor) above 30.

4. Do not rub the cream on your face.

5. Do not use steroid creams.

6. Continue treatment for at least one month.

7. Facewash or soaps are not useful and are not considered medications.

8. Do not touch acne and avoid pressing it.

I suggest you use

1. Gel. Clindamycin (Clintop, Faceclin, or Clear Gel) cream. This is more preventive and curative. So I suggest my patients use it all over the face in the morning.

2. Gel. Adapalene (Adalene or Differin) gel for acne. This should be applied at night.

3. Tab. Doxycycline (Lactose monohydrate, yellow iron oxide) 100 mg once a day.

For dark spots, I suggest you to:

1. Avoid direct exposure to sunlight as much as you can. Wear sunglass, and caps and cover the exposed part of the body all the time.

2. Use sunscreen thrice a day at 10 AM, 12 PM, and 3 PM all over the face.

3. Sunscreen SPF (sun protection factor) should be around 50.

4. Use anti-pigmentation cream over pigmented parts like Kojic acid, Hydroquinone, or Depiwhite cream.

5. If spots are deep and too much then, the ideal treatment is Glycolic acid peels. It may require six to eight sittings of Glycolic acid peels.
