Hi doctor,

My nephew, who is 24 years old, is suffering from a high fever (103 to 104) since last night. He is not getting a dry cough or any symptoms of COVID-19 and not having any shivering also. We got his blood tested, and I am enclosing the blood test report.

Please advise for further course of action.

Thanks for the query. I can understand your concern. According to your statement, your nephew has been suffering from a high fever since last night.

I have gone through his blood reports (attachment removed to protect patient identity) and shown that his platelet count is below normal. Her SGPT (serum glutamic pyruvic transaminase), SGOT (serum glutamic-oxaloacetic transaminase), ALP (Alkaline phosphatase), Neutrophils, and Uric acid counts are above normal, and HDL (high-density lipoprotein), Phosphorus, and MCHC (Mean corpuscular hemoglobin concentration) are below normal.

As her platelet count is below normal, so I think she may suffer from dengue.

She should undergo some investigations like antigen or antibody test for COVID-19 or RT-PCR (reverse transcriptase-polymerase chain reaction) for COVID-19 to exclude COVID-19 infections.

In the meantime, she can take capsule Cefixime (400 mg) twice daily for 10 to 14 days, and fever or pain, take only Paracetamol. Do repeat her CBC (complete blood count) report with platelet count within 48 to 72 hours.

Thanks for joining us again. Hospitalization will depend on her health condition. Suppose her platelet count is decreasing day by day, below 50x10^3 per microliter of blood, and symptoms of thrombocytopenia (below normal count) like easily bruising (purpura), pinpoint (rash or petechiae ), bleeding from gums or nose, blood in stools or urine, etc., or platelet count is below 10x10^3 per microliter of blood. In that case, she should be admitted to the hospital immediately for plasma exchange. Again, monitor her oxygen saturation rate by pulse oximeter routinely. If her oxygen saturation drops below 93%, then she should be hospitalized for oxygen inhalation.
