I have reviewed your attached X-ray (attachment removed to protect the patient's identity). The decay approximates your nerve layer, and the treatment depends on your symptoms. Do you have tooth pain and sensitivity to hot or cold foods? You require a root canal treatment if you have pain or sharp sensitivity. If there is no sensitivity or pain, a filling is generally done.

The tooth has three layers, outer enamel, dentin, and innermost nerve layer. If decay is in the enamel and the topmost dentin layer, a filling will be done, but if the decay progresses and touches the nerve layer, root canal treatment is the only option. But if the decay is in the deepest part of the dentin, the patient's symptoms will be considered to decide the better treatment option. If there are no symptoms, you may require a deep dentinal filling. However, the deep dentinal filling has great odds of becoming symptomatic in the future. Therefore, if it causes pain in the future, you will require a root canal.
