Hi doctor,

My three-year-old daughter has had some rash around her intimate body for one month. It is red, dry, itchy, and causes little pain to her. Sometimes it is better, and sometimes it gets worsened. My pediatrician suggested Aquaphor baby diaper rash paste, but it did not help. Kindly help.

I understand your concern. I suggest the following. Apply Desonide cream at night for ten days. Use Dermocalm lotion (Calamine and liquid paraffin) immediately after bath for ten days. Avoid wearing diapers for some time. Keep the area clean and dry. Take Syrup Atarax (Hydroxyzine Hydrochloride) 3 ml twice daily for five days. Avoid occlusion or friction. Avoid scratching and rubbing. Kindly revert in case of further queries. Thank you.

Thank you for the reply. My daughter needs a diaper at night only. Could you please say what this is? Is it a diaper rash? And why did she get it? Or from what? Kindly help.

I understand your concern. It is an allergic rash with frictional diaper dermatitis. It occurs due to contact with ammonical components (urine) in the diaper area and friction or occlusion. I suggest the following. Keep the area clean and dry. Apply Lactocalamine lotion two to three times to soothe the lesion. Also apply Mupirocin (Bactroban) ointment in the evening for ten days.
