Patient's Query

Hi doctor,

I have been having breathing difficulty for more than two years. It is as though something is blocking me from inhaling or exhaling. I have taken ECG, sugar test, thyroid test, chest X-ray, and heart ultrasound sound scan, and everything seems normal. But my problem keeps worsening, and everyday living seems tough. Why is this happening?

Note:

I have allergy rhinitis condition (most of the time severe headache and blocked nose or congestion). Also, I think I have anxiety or depression as sudden tears roll out often.

Please help.

Patient's Query

Hi doctor,

Thank you for your reply.

I am having breathing difficulty(as though something is blocking me from inhaling or exhaling). I have a loss of appetite. I eat meals only once a day. Also, I have mood swings, mostly low mood, and loneliness. Crying makes me feel better, but it is difficult to cry as my nature is not like that. I also have chest tightness, so lying down makes me feel better. And I get a lot of knuckles in the leg when I do that. I do not generally share my problems with others and talk to myself a lot. This is because my husband and his family subjected me to domestic abuse for the past ten years.

Please help.

Hi,

Welcome back to icliniq.com.

I am sorry about the problem you mentioned in the query

Your ECG (electrocardiogram) and 2D ECHO (two-dimensional echocardiogram) reports are normal (attachment removed to protect patient's identity).

Low mood and loneliness are psychological symptoms that need careful counseling about it. Maybe feeling that something is blocking you from inhaling or exhaling could be related to psychological but diseases related to the respiratory system should be excluded first. However, I have a few questions,

1. Do you have episodes of asthma?

2. Did you have any respiratory problems in the past?

3. How are you uplifting yourself from low mood and past history?

Kindly reply to the questions so that I can guide you with counseling regarding it.

I hope this helps.

Thanks and regards.

Patient's Query

Hi doctor,

Thank you for your reply.

Yes, I have allergy rhinitis, and I often take the tablet Paracetamol for congestion, headache, and a blocked nose. It is becoming difficult to uplift my mood off-late, and listening to devotional music is my only option now. Even I feel frustrated doing that sometimes, and I try playing mobile games to let out my stress. I like playing a game that involves breaking things recently.

After my episodes of abuse, I approached the police here once, and they suggested family counseling. I attend those sessions once in a while, but I still do not feel that it is also helping. I have come to a point where all hope is lost, and no solution is possible. I am concerned about my kids and their welfare.

Please help.

Hi,

Welcome back to icliniq.com.

If you often get allergic rhinitis first precaution is to identify the possible allergens and prevent exposure to the allergen. Allergic medication like Levocetirizine (antihistamine) would be more useful than Paracetamol (non-steroidal anti-inflammatory drug) in controlling allergic rhinitis episodes. So, I suggest you to take a tablet of Levocetirizine before bedtime if you get an episode of allergic rhinitis. There is always a solution. The real problem lies in our thinking patterns.

Try to observe thinking and thoughts that constantly arise in your mind, requiring attention. Attend the counseling session. Observe in your thinking that what is bothering you is about your kids and their welfare, financial security, etc. Remember, everyone is struggling because their thinking pattern is distorted; the only difference is that some have psychological manifestations and others do not.

Kindly consult a specialist, talk with them and take the medications with their consent.

I hope this helps.

Thanks and regards.

Patient's Query

Hi doctor,

Thank you for the response.

Noted on the Levocetirizine part, but nowadays, I find it quite difficult to sleep at night, so I feel restless during the day. I had a mild dose prescribed earlier that made me sleepy throughout the day, but I was given only for a month as the psychiatrist suggested to consult someone else in the place I live. I always think about why this happened when I tried to do the best for my family, even at the cost of my own happiness and health. I feel so stupid for being that way. Now my health is very bad, and I am going through pain again.

I am confused if one should be good when people around you take you for granted. I served my husband on top of all the suffering, yet I did not receive any support from him, his care, concern, or understanding. He was not even ready to give me pocket money.

Now I have learned to let him go, and my kids started to work, though health is the biggest concern.

Please give me a solution.

Hi,

Welcome back to icliniq.com.

I will try my best to guide, motivate, and uplift you.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

I just expressed what I have been feeling. I think the basic feeling of not having a companion to listen to, care and understand has gone deep inside me. I have realized that I am going through all this because of my love for him despite all the suffering. The attachment was the reason I was just unlucky. So I have started enjoying being alone.

Thank you.

Hi,

Welcome back to icliniq.com.

Everyone on this planet needs good company to share basic feelings with someone. I do not know how you would feel, and it is tough for me to measure the depth of your psychological suffering. You definitely need psychological support, and lack of psychological support manifests in somatic symptoms. You should have realized his behavior very early of inattention towards you.

Try to find meaning in your life and a goal to dedicate your life to. It may be self-accomplishing something greater or for your child or something else. The goal in life is essential to roll life on a positive and meaningful track. Without any goal in life, our mind gets occupied with thoughts that are not productive, and life becomes more negative. Try to find a goal or meaning in your life that could give you purpose to move on from the past and fill the present moment with a positive attitude. This present moment with a positive outlook would become past memory and replace the bitter past memory.

However, I suggest you to meditate in your spare time. Learn to live alone and quietly. Your energy would gather to overcome negative thoughts and highlight how to live positively.

I hope this helps.

Thanks and regards.

Patient's Query

Hi doctor,

Thank you for your advice.

Your message gave me a push to start my day. Whatever you have mentioned is very true. I am trying to do the same. When I realized my husband and his attachment or fear towards his parents to the extent of torturing me, I completely turned my focus to the welfare of my kids. But unfortunately, they also had allergy issues every other day with so many behavioral tantrums, and I could not lead a peaceful life. That is when I tried hard to detach myself from everything for my health.

I am just wondering if I should check for respiratory issues. What test do you suggest? I wonder if this difficulty breathing (feeling of blocking when inhaling or exhaling) is related to that. Because I had severe palpitations in the morning and I managed to switch on the humidifier with eucalyptus oil, I felt better. That is why I am requesting you suggest a test to check for blockage in the windpipe or lungs.

Please give your suggestions.

Hello,

Welcome back to icliniq.com.

I think palpitations and respiratory issues are linked with your psychological disturbance. However, I suggest you to get a chest X-ray and spirometry test done. These tests will help determine if there is any obstruction in the respiratory passage during inhalation or exhalation. In your ECG, there was no increase in heart rate, but you have palpitations on and off, which means it is due to anxiety. Do you sleep adequately daily?

I hope this helps.

Kindly follow up with the reports.

Thanks and regards.

Patient's Query

Hello doctor,

Thank you for the reply.

I have done a couple of chest X-rays in the past, but the doctor told me it was normal. I will figure out the other test you recommended. And sleep is becoming a big issue these days. I feel restless, and I have clogged thoughts. Also, I feel tired, sleepy, and breathless in the morning. I suspect there is some airway inflammation that needs to be diagnosed. Even my kids were prescribed inhalers for some time when I took them to a pediatrician here, but later I stopped giving them. Do inhalers (blue and orange colored ones) contain steroids?

Please answer.

Hi,

Welcome back to icliniq.com.

A spirometry test would help to identify whether there is any airway inflammation and the problem with inhalation and exhalation. Auscultating over the lung area would also give a clue to treating doctors in formulating a differential diagnosis. Most inhalers contain steroids, but they also contain other medications.

Are you currently taking any medication for insomnia?

I hope this helps.

Kindly follow up with the answers.

Thanks and regards.

Patient's Query

Hi doctor,

Thank you for your reply.

No, I am not taking any medication for sleeplessness. And I have a psychiatric appointment next month. Here all the appointments are quite delayed as government-based hospitals operate that way (private ones are quite expensive compared to government hospitals). I inquired about the spirometer test today. I need to go through the hospital only, as clinics do not support that.

Thank you.

Hi,

Welcome back to icliniq.com.

You should do the spirometry test as it will help you rule out if there is any obstruction during inhalation and exhalation of the breathing process. I suggest you to schedule an appointment with a psychiatrist to guide you on how to change your attitude and outlook on the present problem.

I wish you to become psychologically healthy very soon.

I hope this helps.

Thanks and regards.
