Hello doctor,

I am having issues with hearing, pain, pressure, and sensitivity to noise. Can slough in ear be painful? I am currently using Topamax, Allegra D, Baby Aspirin, Tylenol, Aleve, and Ibuprofen.

Sorry, I need more details but I will comment on the pictures as clear as possible. (attachment removed to protect patient identity). The first picture shows a lot of shaded superficial layers of the skin. This can be painful. This may be a dermatological disease like eczema. I still do not know which ear is it, right or left? The last picture seems to be of the left ear. The drum is intact, no perforation, the white patches in the drum are called tympanosclerosis which is deposition of calcium granules within the drum tissue. This can happen spontaneously or after healed drum perforation and drum surgeries.

It is my right ear. I am trying to figure out if that is possibly a hole in my ear drum. I have a lot of pain. I am just unsure how the picture within a short duration looks so different with no trauma. Is there anything that can be done?

Yes, the two pictures are different but I think that there is no perforation, it is just an external ear skin inflammation. Ear perforation with pain must be accompanied by ear discharge and pus. As you can see that your ear is completely dry. There is a simple test to examine your eardrum for the presence of a perforation, it is called Valsalva test. Close your mouth and your nose and then blow, if you felt an air coming out from your ear it means that the drum is perforated. Use antibiotics plus corticosteroid ear drops three times daily besides your ordinary medications. Keep your ear dry and away from the water.
