Hi doctor,
I feel like I have erectile dysfunction because I get hard spontaneous erections with stress in my penis, but I cannot maintain hardness in masturbation. Please help.

I can understand your concern about your symptoms.
However, according to your complaints, you may have erectile dysfunction. Reasons for erectile dysfunction are stress due to work or family-related, night duties, depression, varicocele, fear of failure, having one episode of erectile dysfunction once may make you feel it lasts forever, low self-esteem (chronic masturbation), homosexuality, religious constraints, smoking and alcohol, long-term illness, cardiac disease, thyroid, kidney disease, or any other disease with long-term medication, negative attitude towards your partner, hormonal issue, thyroid issue and low testosterone, long-term drug usage, diabetes and hypertension.
Practice regular exercise and yoga. You need to check the Doppler of the penis, USG (ultrasonography) of the pelvis and scrotum, blood pressure, RBS (random blood sugar), free T3 (triiodothyronine), free T4 (tetraiodothyronine), testosterone, and TSH (thyroid-stimulating hormone).
Please share more details regarding the above reasons, and we can work on diagnosis and treatment. For a temporary solution, I suggest 5 mg of Tadalafil twice a day.
