Patient's Query

Hello doctor,

I am a 20-year-old experiencing erectile dysfunction, despite being healthy and active with regular gym sessions four times a week. I do not have morning erections, only occasional ones while sleeping (approximately once every two weeks). Although I can achieve erections when masturbating while lying down, they quickly fade when standing or without manual stimulation. This issue has persisted for 8 months and is taking a toll on my mental well-being. I should mention that I am a porn addict and have been masturbating frequently, especially since the onset of these symptoms. I have also been dealing with penile pain when flaccid, a lump on my penis, and a slight bend during erections, although there is no curvature when fully erect. Concerned about Peyronie's disease, I consulted a urologist who palpated my penis and found no plaques, ruling out that condition. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

Considering your description, it is possible that you are experiencing symptoms of erectile dysfunction (ED), characterized by difficulty achieving or sustaining an erection suitable for sexual activity.

Several factors could contribute to your ED, including:

  1. Porn addiction and excessive masturbation: These habits might lead to penile desensitization, making it challenging to attain or maintain erections.
  2. Stress and anxiety: Psychological factors can exacerbate ED, creating a cycle of performance anxiety.
  3. Penile pain or discomfort: This may impede your ability to achieve or sustain an erection.
  4. Penile lump or curvature: Though Peyronie's disease was considered, it has been ruled out by your urologist.

To address your ED symptoms effectively, I would suggest you collaborate with a healthcare provider. They can conduct a thorough evaluation, including physical examinations and blood tests, to rule out any underlying medical conditions contributing to your symptoms. Based on this evaluation, appropriate treatments and lifestyle adjustments may be recommended. Some of the treatment options include:

  1. Medications: Phosphodiesterase-5 inhibitors such as Viagra (Sildenafil citrate) or Cialis (Tadalafil) to enhance blood flow and aid in achieving and maintaining erections.
  2. Psychological counseling or therapy: To address any anxiety or stress linked to your ED symptoms.
  3. Lifestyle changes: Such as quitting smoking, moderating alcohol consumption, and maintaining a healthy weight, which can improve cardiovascular health and reduce the risk of ED.
  4. Penile rehabilitation exercises: Pelvic floor exercises or vacuum pump therapy to enhance erectile function over time.

ED is a common issue affecting many men, and effective treatments are available. With appropriate care and support, managing symptoms and improving sexual health are achievable.

I hope I have addressed your concerns.

Take care.
