Hi doctor,

I am a 41-year-old female with a weight of 55 kilograms and a height of five feet. I have a fever and severe headache for three days. I vomited twice and saw some sticky phlegm. I do not have a fever after taking Paracetamol every four hours and stopped taking it when my fever was gone. But still, I have headaches and I cannot sleep at night. Kindly advice. What to do? I did a rapid test on the second day of fever and it showed negative results.

Thanks for your query. If you need to confirm, please undergo RT-PCR (reverse transcription-polymerase chain reaction) test, not rapid test. Additionally, you can do the following investigations. You can do CBC (complete blood count), LDH (lactic dehydrogenase), CRP (C-reactive protein), renal function tests such as serum creatinine, eGFR (estimated glomerular filtration rate), serum BUN (blood urea nitrogen), electrolytes, liver function tests such as ALT (alanine transaminase), AST (aspartate transaminase), Hba1c (Glycated hemoglobin), and ECG 12-lead (electrocardiogram). You can check oxygen saturation using a pulse oximeter twice daily.

You can take tablet Zinc 50 mg one tablet thrice daily for 15 days, tablet Vitamin C 500 mg one tablet twice daily after lunch and dinner for 15 days, tablet Ivermectin 12 mg one tablet once daily for three days, capsule Doxycycline 100 mg one capsule twice daily after lunch and dinner for five days, tablet Famotidine 40 mg one tablet twice daily after lunch and dinner for 15 days, tablet Xyzal (Levocetirizine) one tablet once daily for seven days, tablet Fluimucil (Acetylcysteine) 600 mg one tablet once daily for seven days, capsule Econorm (Saccharomyces boulardii) 250 mg one capsule twice daily for five days, tablet Cinitapride 1 mg one tablet thrice daily for five days, and tablet Paracetamol 650 mg one tablet on SOS basis. Most of the above medicines are given in generic names, you need to check the locally available brands and take them accordingly. Please revert after tests.

Investigations to be done

Do nasal and nasopharyngeal swabs for SARS-COV2 (severe acute respiratory syndrome coronavirus 2) virus by RT-PCR (reverse transcription-polymerase chain reaction), blood tests such as complete blood picture, LDH (lactic dehydrogenase), CRP (C-reactive protein), renal function tests such as serum Creatinine, eGFR (estimated glomerular filtration rate), serum BUN (blood urea nitrogen), and electrolytes, liver function test such as ALT (alanine transaminase), AST (aspartate transaminase), HbA1C (glycated hemoglobin). You can check oxygen saturation using a pulse oximeter twice daily (note readings below 94 for SPO2). Do ECG- 12 lead (electrocardiogram) and X-ray chest.

Differential diagnosis

You seem to have a bacterial upper respiratory tract infection.

Probable diagnosis

You seem to have an upper respiratory tract infection caused by SARS COV2 (severe acute respiratory syndrome coronavirus 2) or any other virus.

Treatment plan

Follow as given above.

Preventive measures

Have warm water intake for drinking and do steam inhalation twice daily.

Regarding follow up

Have a follow-up after doing tests.
