My 3-year-old had a fever four days back and since then has had foul-smelling vomit and diarrhea.

Considering the duration of the fever I would suggest getting the child evaluated physically by a pediatrician with blood investigations like CBC (complete blood count), CRP (C-reactive protein), etc. Meanwhile, you can do the following:

Consult your specialist doctor, talk to them, and take medications after their consent.

  1. If the temperature is 100 degrees Fahrenheit or more, you can give syrup Crocin DS (Paracetamol) 240 mg/ml (dose as per the weight of the child) SOS four to six hourly.
  2. If the temperature is 102 degrees Fahrenheit or more, you can give syrup Ibugesic Plus (Ibuprofen and Paracetamol) (dose as per the weight of the child) SOS six to eight hourly.
  3. Give syrup Ondem (Ondansetron) (dose as per the weight of the child) thrice a day for three days (to be taken half an hour before food or water).
  4. Syrup Nutrolin B (Lactobacillus, Niacinamide, Pyridoxine, and Folic acid) 5 ml twice a day for five days. It should be kept in the fridge.
  5. ORS (oral rehydration therapy) 10 ml/kg with each vomitus or loose motion.
  6. Watch for dehydration, lethargy, and refusal to feed.
