Hello doctor,

I have pain in my fingers hands and feet in the morning when I wake up. After I do some exercise the pain reduces. What must I do? I am currently on T4 levothyroxine.

I have gone through your reports (attachment removed to protect patient identity), it shows that you have a high ANA (antinuclear antibodies). These are antibodies related to arthritis. I suggest that the pain in your fingers and toes may be associated with the same. In such cases, we generally put you on a medication called Plaquenil (Hydroxychloroquine). This is a medication for arthritis that needs to be taken regularly. As the medication is continued, the pain will come down. I also suggest that you get other tests done like ENA (extractable nuclear antigen) and urine routine examination. Your tests also show that vitamin D levels in your blood are low, so you need to take a supplement for calcium and vitamin D.

Yes, these are also positive. I suggest starting Plaquenil, that should help you. DVT (deep vein thrombosis) may also be related to this disease. You should feel better, but it will take time a few weeks.

Then you can start Methotrexate 10 mg once a week along with tablet Folic acid 5 mg on alternate days. Check your CBC (complete blood count) and liver function test after one month.
