Hello doctor,

I was getting heartburn every day for a long time. I used TUMS most of the time. Currently, during or immediately after eating, I feel gas that wants to come out of me without feeling stomach pain or bloating. Most of the time, after drinking water or eating, I have gas incontinence without feeling it. I feel the smell of gas without feeling the passing of the wind. My wife and some friends also told me they sometimes smell it, increasing when eating, drinking, and before defecating. Knowing that when I feel the urge to make gases, I hold them back. Sometimes I can control the gases and stop them when I feel them.

You are suffering from upper abdominal symptoms like heartburn and gas-trapped feelings and lower abdominal symptoms like excessive gas formation and passing out.

1. Regarding heartburn, recently you were treated for H. pylori gastritis, so you should take Antacid syrup like Gaviscon 2 tablespoons full after every meal or as per need, you can use this for a couple of weeks.

2. Meanwhile, try to have small frequent meals, do not fill your stomach at once, do not lay down immediately after meals, and your last meal should be two hours before bed.

3. Moving on to excessive gas production and passing out, antibiotics frequently disturb normal gut bacterial flora, causing dysbiosis (disturbance in normal healthy bacteria), which can present as flatulence. For this, you should take probiotics or replace bad bacteria in your gut with good bacteria. You can take commercially available probiotics or even natural probiotics like yogurt daily. You mentioned your hemorrhoidal surgery. I hope you must be having good voluntary control over your stools. If yes, then there is nothing to worry about flatulence. It will go away with good probiotics.

5. Try to take home-cooked meals, and avoid excessive drinking of alcohol or carbonated drinks.
