Hello doctor,

I was on capsule Fluoxetine (SSRI) for just over six years for depression, but I decided to stop the medication last year as I thought I was now in a good position to deal with my issues drug-free, unfortunately, after a year, my issues resurfaced, so I decided to go back on the medication, but for some reason, the medication has been causing eye and facial pain on the right side of my face.

I have a warm dull ache in my eye, sometimes in the corner of my eye, and what feels like pain at the back of my eye (it is quite difficult to pinpoint where the pain actually is) and my eye also becomes bloodshot red and I have pulsating sensations in the cheekbone, temple, and the forehead area, I have also developed nasal congestion. I stopped the medication for two weeks, and the issues subsided, so I tried the medication again, and unfortunately, the issues have resurfaced, I have tried other SSRIs, but they have also caused the same problems. I have been to my General practitioner, but he does not know why the medication is causing it. I have had a Computed tomography scan which was assessed by an ENT doctor who ruled out any sinus issues. I have come to the possibility that Serotonin may be the culprit, but obviously, I am not an expert. Do you have any idea as to what could be happening?

The phenomenon that you described after the consumption of capsule Fluoxetine in your eye is compatible with an angle closure attack. It causes red eye, eye pain, headache around the ophthalmic area, tearing etc. Accordingly, I strongly recommend you not to use the drug. You need to visit an ophthalmologist (eye doctor) to assess your eye-ball pressure (intra-ocular pressure) and evaluate your presence of glaucoma.

It is a bit odd that a red eye was regarded as completely normal by an ophthalmologist. Nevertheless, if it is confirmed that your intra-ocular pressure is always normal and the problem arises just after capsule Fluoxetine consumption, you should not continue this drug. Moreover, a Brain MRA (magnetic resonance angiography) needs to assess any vascular problem in your arterial branches to the eyes.
