You seem to have a severe allergic problem with associated sinusitis. I have tried to explain the cascade of events in a simple manner. Exposure to dust initiates the reflexes such as sneezing to remove the offending agent. If this fails, mucus cells in the nose start to produce loads of mucus to trap dust. The mucus is either blown out due to mucociliary activity or it gets pushed into the throat entering a new zone. The throat reacts by activating cough impulses to push out the mucus. If this fails, a local inflammation around the throat is formed and it leads to symptoms such as a lump in the throat and repeated feeling to clear the throat. This is the general cascade that occurs in most people. Sinusitis usually occurs secondary to allergy. There are other reasons to have sinusitis such as hypoplastic frontal sinus, narrow frontal outflow tract or frontal recess, or sinonasal polyposis. Plain use of antibiotics does not help to provide relief but it just keeps off for a while. We need to rule the cause of sinusitis and then appropriate treatment has to be taken to treat them. Taking precautionary measures helps to resolve it. You can follow these measures. Wear N95 masks at all times. It does not allow any dust or irritative substance entering into the nose. You can take tablet Sinarest (Chloramine maleate, Paracetamol, and Phenylephrine) one tablet two times a day after food for 15 days, tablet Allegra (Fexofenadine) one tablet two times a day after food for 15 days, tablet Zerodol 4 mg (Aceclofenac with Thiocolchicoside) one tablet two times a day after food for five days, and tablet Clarithromycin 500 mg one tablet two times a day after food for 15 days. Use Duonase nasal spray (Fluticasone propionate with Azelastine) two puffs two times a day for three months. You can take tablet Wysolone 10 mg (Prednisolone) one tablet after breakfast for a week. Have steam inhalation two to three times a day. Do salt water gargling eight to ten times a day for a month. Do not smoke or drink alcohol as it slows down the mucociliary clearance mechanism. Follow these precautionary measures to have maximum relief. If there is no relief from the symptoms even after taking treatment, then it is important to take a non-contrast CT (computed tomography) to find the cause. Anatomical variations and extend of inflammatory load are some things seen in the CT reports. Surgery can be required in some cases. These symptoms can come back even after undergoing surgery if these precautionary measures are not followed properly.
