Hi doctor,

My sister's right hand and wrist are aching continuously since the past few weeks. The pain has increased now. She says that under her little finger, the pain is comparatively greater than in the rest of her hand. Her palm has turned black and hard.

Your sister may have impeding gangrene due to blocked blood vessels in her hand. She should undergo right upper limb arterial doppler. If the reports show obstruction, then you should immediately consult a cardiovascular surgeon. If possible, please upload a photo of the hand.

    From the attached photo (attachment removed to protect patient identity) shows the index and little finger of the left hand, but you said the problem is in the right hand. Please consult radiologist and do doppler of the affected hand. I suggest the following medications (consult with a specialist doctor, talk with him or her and take medicines with their consent),

    1. Tablet Stiloz (Cilostazol) 50 mg, twice a day.
    2. Tablet Eslo (Amlodipine) 2.5 mg, once a day.
    3. Tablet Trental (Pentoxifylline) 400 mg, once a day.
    4. Tablet Ultracet (Tramadol and Paracetamol), twice a day.
    5. Capsule Cyra D (Domperidone and Rabeprazole), on an empty stomach.
